Position Overview:
We are seeking a passionate and organized Licensed Practical Nurse (LPN) to step into a Program Coordinator role at a comfortably sized residential home supporting one medically complex youth.
This is a leadership position overseeing a team of dedicated Health Care Aides (HCAs) — perfect for an LPN who loves mentoring, collaborating, and leading high-quality care delivery.
You’ll combine clinical skill with team leadership, helping create a supportive, structured, and positive environment.
Key Responsibilities:
- Oversee daily operations of the home, ensuring exceptional care and safety practices.
- Lead, coach, and support a small team of Health Care Aides (up to 8 direct reports).
- Develop, monitor, and adjust individualized care plans.
- Coordinate appointments, medication administration, seizure protocols, and communication systems (low- and high-tech AAC devices).
- Train staff on proper medical procedures (lifting, seizure intervention, G-tube feeding, toileting schedules).
- Maintain professional, compassionate communication with youth’s family, case workers, and external medical teams.
- Model and enforce trauma-informed, respectful caregiving practices at all times.
- Complete documentation, reports, incident reviews, and shift summaries accurately and timely.
- Collaborate with senior leadership to continuously improve care quality.
- Operate Wheel Chair van when needed
Qualifications:
- Current Licensed Practical Nurse (LPN) registration in BC
- 5+ years experience in residential care, long-term care, pediatric care, or complex medical settings.
- Prior supervisory experience or team lead experience strongly preferred.
- Strong knowledge of seizure protocols, G-tube feeding, lift and transfer safety, and toileting routines.
- Exceptional communication skills — able to lead a team while maintaining empathy and patience.
- Current CPR/First Aid certification (We can also renew).
- Clear criminal record check required. Must be able to able to pass a Ministry (MCFD) clearance and maintain it.
- Valid BC Drivers License
Bonus if you have previous experience supporting youth with complex developmental or neurological needs.
